State Tax Representation
Most states require a form to be signed to have representation. However, some states use the federal Form 2848 as legal authority for your representation. In other words, you have to sign only one form for representation in all jurisdictions.
For instance, Michigan requires Form 151, AUTHORIZED REPRESENTATIVE DECLARATION. To represent a company in Unemployment issues in Michigan, a Form 1488 must be signed. Some state forms require only one form for both state income tax and employer tax issues as well as unemployment.
